- Threads found damaged, crossed or pulled on removal

- Cracked body after over-tightening
- Loose fit that will not hold the mating part square

- Renew the sealing washers or o-rings at both joints rather than over-tightening
- Check the threads in the housing before fitting - a damaged female thread will take the new adapter with it
