• Clamp bolts found slack, or their holes worn
• Bore and shaft found fretted or scored on removal

• Measure the shaft before ordering - this coupling is bored 45 mm, and a coupling that does not match the shaft exactly cannot be made to grip safely
• Check and reset the engine-to-shaft alignment after fitting; a clamp coupling on a misaligned shaft will fret itself loose and wreck the stern gear
• Renew the clamp bolts and their locking, and re-check them for tightness after the first few hours running
